Support Coordination That Works for You
We offer two levels of NDIS Support Coordination — both delivered with independence, expertise, and genuine care for your goals.
What Is Support Coordination?
Support Coordination is a capacity-building support funded through your NDIS plan. A Support Coordinator helps you understand your plan, connect with the right providers, and build the skills to manage your supports independently over time. At Lugenah Care, we are completely independent — we are not affiliated with any service provider, so our advice is always in your best interest.
Support Coordination (Level 1)
Helping you understand and implement your NDIS plan
Support Coordination (Level 1) is designed for participants who need assistance understanding their NDIS plan and connecting with appropriate supports. Our coordinators work with you to put your plan into action, navigate the NDIS system, and build your confidence to manage your own supports over time.
What's included:
- Understanding your NDIS plan and funding categories
- Identifying and connecting with suitable service providers
- Coordinating services and supports across your plan
- Preparing for and attending NDIS plan reviews
- Building your capacity to self-manage supports
- Monitoring your plan budget and utilisation
Best suited for:
Participants with straightforward support needs who want help getting started with their NDIS plan and building independence.
Specialist Support Coordination (Level 2)
Intensive, specialist support for complex situations
Specialist Support Coordination (Level 2) is for participants with more complex needs or circumstances that require a higher level of expertise. Our specialist coordinators have advanced knowledge of the NDIS, mental health, disability, and complex service systems — providing intensive support to resolve barriers and ensure your plan is working effectively.
What's included:
- Intensive coordination for complex or high-risk situations
- Crisis planning and management support
- Navigating complex service systems (mental health, justice, housing)
- Developing and implementing robust support plans
- Liaising with multiple agencies and stakeholders
- Supporting transitions (hospital, justice, aged care)
Best suited for:
Participants with complex support needs, multiple diagnoses, or circumstances requiring specialist knowledge and intensive coordination.
